You no longer have to submit paper forms to set up withholdings of additional funds through payroll deduction. Withholding requests are now set up over the phone by speaking to one of our Member Services Representatives. To make a withholding request, please call 323/993-8888 or 888/FLEX-401K (Option 1) during our regular business hours. You will need to provide the Representative the following information:

• Your Social Security Number
• Your 401(k) Plan 5 digit PIN
• Your Name
• The Employer or Payroll Company where you would like the withholdings to be set up
• The withholding type
  • 401(k) – Pre-tax
  • Roth – After tax
• The withholding percentage

If you are on email delivery, you will receive the confirmation within about 10-15 seconds after making your elections. If you are on paper delivery, your confirmation will be mailed within 1-2 business days. If you are not on email delivery, you may wish to sign up for electronic delivery before calling in your elections at ei401kplan.com/email to receive your confirmation immediately and receive a discount on your administration fees. Once you have made the withholding request with our Member Services Department, your employer will be notified of your withholding request the following business morning.
